These Terms and Conditions (“Terms”) govern the use of services provided by Shooting Medicals Limited (“we”, “us”, “our”). By booking a firearm medical examination with us, you (“you”, “the client”, “the customer”) agree to these Terms. Please read them carefully before making a booking.
1.1. Shooting Medicals Limited provides firearm medical assessments carried out by GMC-registered doctors.
1.2. The service includes:
Provision of an Access Information Request form for you to submit to your GP to obtain your full medical records.
Provision of a Customer Information Form to be completed by you with your personal details.
Review of your official GP medical records (sent directly from your GP practice) to prepare the required medical report.
1.3. Our doctors can only base their assessment on official GP medical records received directly from your GP practice. We cannot use records or information submitted by you personally.
2.1. You are responsible for:
Completing and returning the required forms to us in full.
Submitting the Access Information Request to your GP practice.
Ensuring your GP practice releases your medical records to us.
2.2. We cannot be held liable for delays caused by GP practices in releasing medical records.
2.3. If your GP practice refuses to release your records, it is your responsibility to escalate the matter, including lodging a complaint with the practice or relevant regulatory body.
3.1. We may, at our discretion, assist in following up with your GP practice (e.g. via telephone or email).
3.2. Such assistance is not a guaranteed or mandatory part of our service, and no liability is accepted if records remain unobtained.
4.1. The fee for a firearm medical assessment is £60, payable at the time of booking.
4.2. Refunds are only available within seven (7) calendar days of booking, provided no medical has been completed.
4.3. After the 7-day period, no refunds will be issued, even if:
Your GP practice refuses to release your records.
You are unable to complete the medical with us.
You choose to obtain the medical elsewhere.
5.1. Medical records received from your GP are used solely for the purpose of conducting your firearm medical assessment.
5.2. We retain your medical records for up to six (6) months after completion of the assessment. Records may be retained to allow us to respond to queries from the police or other relevant authorities.
5.3. After this retention period, your records will be securely deleted or destroyed in accordance with data protection law.
6.1. We comply with all applicable data protection legislation, including the UK GDPR and Data Protection Act 2018.
6.2. Your medical records and personal data will be handled confidentially and securely.
6.3. We will not use your medical records for any purpose other than preparing your firearm medical assessment and responding to lawful requests from the police or relevant authorities.
7.1. Our doctors act independently and in accordance with GMC guidelines.
7.2. The outcome of your medical assessment cannot be guaranteed and will depend entirely on the content of your official GP medical records.
7.3. We reserve the right to decline or refuse to complete a medical report at our sole discretion.
8.1. To the fullest extent permitted by law, Shooting Medicals Limited shall not be liable for any:
Delays in obtaining medical records from GP practices.
Refusal by GP practices to release records.
Refusal by the police to accept a report.
Indirect, incidental, or consequential loss or damage.
8.2. Our total liability in respect of any claim shall not exceed the amount paid by you for the service.
9.1. We reserve the right to refuse service to any client without obligation to provide a reason.
9.2. In cases where service is refused before records are reviewed, we may, at our discretion, issue a refund in line with Section 4.
10.1. These Terms are governed by and construed in accordance with the laws of England and Wales.
10.2. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts of England and Wales.
